چکیده

Because of the frequent occurrence of significant dysrhythmias after Mustard's operation for transposition of the great arteries, electrophysiological studies were performed in i6 patients during postoperative cardiac catheterization. Intracavitary electricalpotentials were recordedfrom the conduction system in all i6patients. In 9 of these patients, the stability of the atrioventricular conduction system during rapid atrial pacing was tested, as well as the subsequent promptness ofpacemaker recovery. The pacemaker recovery time, corrected for cycle length, was significantly prolonged in 4 of 9 patients studied. After atrialpacing, an escapejunctional rhythmpreceded the resumption of sinus rhythm in 3 patients. No intra-atrial block was found between the upper and lower intra-atrial electrograms. In 4 patients, an early electrical activity was recorded inside the pulmonary venous (new left) atrium, above the tricuspid valve. This electrical activity occurred during the T-P interval and was unrepresented on the surface electrocardiogram. No significant abnormalities ofconduction were observed in the atrioventricular node or the His Purkinje system. Furthermore, rapid atrial pacing caused no abnormal atrioventricular block. The abnormal pacemaker recovery time and the absence of conduction disturbances in the atria, atrioventricular node, and His-Purkinje system confirm the hypothesis that injury to the sinoatrial node is the main

Cardiac rhythm and conduction before and after Mustard's operation for complete transposition of the great arteries.

SUMMARY Between 1973 and 1978, 120 patients with transposition of the great arteries had a modification of Mustard's operation designed to avoid damage to the sinus and atrioventricular nodes. Of these, 88 had an atrial baffle alone from which nine hospital and four late deaths occurred (total mortality 14.8%); 32 had additional closure of ventricular septal defect and/or relief of left ventric...

Echocardiographic visualisation of the interatrial baffle after Mustard's operation.

Thirty children aged from 7 weeks to 14 years were examined by echocardiography after Mustard's operation for transposition of the great arteries. Discrete and persitent echoes were noted within the original left atrial cavity and contrast echocardiography was used to establish that these originated from the interatrial baffle. In the presence of caval channel obstruction, caused by malposition...

Exercise ability after Mustard's operation.

Twenty children who were well six to 12 years after undergoing Mustard's operation for transposition of the great arteries were studied. Each child performed a graded maximal treadmill test with measurements of gas exchange and oxygen saturation, and had electrocardiography carried out. Nineteen were also catheterised, and oxygen consumption was measured so that pulmonary and systemic flow coul...

Late development of pulmonary venous obstruction following Mustard's operation using a dacron baffle.

Three patients developed late pulmonary vein obstruction (PVO) following Mustard's operation for transposition of the great arteries. In all three the absence of PVO had been documented by an earlier postoperative cardiac catheterization. At reoperation shrinkage and kinking of the dacron baffle was evident. After Mustard's operation, patients should be observed closely for symptoms and signs o...
